Authorized Contents: The RETURNPAK Cactus® Sink Cartridge Prepaid Return Shipping & Disposal Program is approved for the shipment of four (4) used solid or liquid cartridges. Original manufacturer packaging must be used for this supply code. IF YOU DO NOT HAVE THE ORIGINAL MANUFACTURER PACKAGING FROM STRYKER INSTRUMENTS, DO NOT USE THIS PRODUCT. See Supply-377 or Supply-378 which supplies the necessary packaging.
Restrictions: For use in the lower 48 contiguous states only.
How Supply-376 Works: Once you place your order for the Supply-376, the Veolia customer service team will email you a Return Shipment Authorization Form to complete. Once completed you will send the form back to customer service via fax or email. Customer service will then complete the rest of the form and email you the following documents: Final Return Authorization Form, Proof of Purchase, Receiving Document for our processing facility and a Return Shipping Label.
